Tooth Extraction — Wisdom Teeth — PRF
Surgery at Mikrostomart is not just about tooth extraction. It involves precise procedures using the latest technologies — PRF (Platelet-Rich Fibrin), laser, ozone therapy — which accelerate healing and minimize discomfort.
Each surgical procedure is preceded by tomographic diagnostics (CBCT), allowing us to know exactly what to expect even before the procedure begins.
Most Patients fear surgery because they associate it with painful childhood visits: bleeding, swelling, two weeks of suffering. Modern surgery is a different world — computer anesthesia The Wand, PRF (fibrin from your own blood) for the wound, laser biostimulation LLLT after the procedure. Most Patients do not need pain medications stronger than ibuprofen.
What we do and what we don't
In our office, we perform: extractions (simple and atypical, including impacted wisdom teeth), apicoectomy (microsurgery endodontics), single and bridge implants, bone augmentations (sinus lift, GBR), procedures with PRF and ozone therapy.
What we do not do: major orthognathic surgery (jaw-jaw class), oncological, traumatic. This requires hospital conditions and specialists with dedicated residency. We refer to partner hospital clinics in Opole/Wrocław.
Our philosophy: if I can perform the procedure safely and predictably — we do it in the office. If it requires hospital conditions — we refer honestly. It is not an art to try to do everything ourselves — the art is knowing when it is not our case.
Scope of Surgical Procedures
The range of surgical procedures performed in the office. Each preceded by CBCT diagnostics (tomography) — we know exactly what to expect before the incision.
Wisdom Tooth Extraction
Painless removal of wisdom teeth, including those completely impacted in the bone. CBCT diagnostics + PRF = quick healing.
Tooth Extractions
Atraumatic removal of teeth with maximum preservation of bone and gum for future implantation.
Bone Augmentation
Bone reconstruction with bone substitute material prior to implantation. Sinus Lift.
Gum Procedures
Gum plastic surgery, recessions, vestibule deepening. Aesthetics and function of soft tissues.
Apicoectomy (microsurgery endodontics)
Resection of the root apex + retrograde fill. A rescue for teeth with recurrent infection after RCT. Performed with Er:YAG laser — minimal invasion, quick healing.
Frenectomy (frenulum clipping)
Correction of a too short frenulum of the lip or tongue. In children: speech problems, sucking. In adults: gum recession. Er:YAG laser = no bleeding, no stitches.
Wisdom tooth extraction — workflow
Wisdom teeth (third molars) are the most common surgical procedure in the office. The procedure is predictable and safe provided that accurate diagnostics are performed.
PRF Technology — Natural Healing Acceleration
PRF (Platelet Rich Fibrin) is a membrane produced from the Patient's blood just before the procedure. We take 10-20 ml of blood, spin it in a special centrifuge — in 12 minutes we obtain a yellowish gelatinous membrane rich in platelets and growth factors.
The membrane is introduced directly into the wound after extraction or during bone augmentation. Platelets release growth factors (PDGF, VEGF, TGF-beta) that actively stimulate tissue regeneration. Without PRF, the wound heals in 2-3 weeks — with PRF usually 7-10 days.
100% natural — no chemical additives, no risk of allergic reactions. The material comes from your own blood. The procedure is approved by most European surgical societies. In our office, we routinely offer PRF during wisdom tooth extractions + any implant placement.
When we particularly recommend PRF
- After extraction of impacted wisdom teeth (especially lower ones) — reduction of swelling and postoperative pain
- Any implant placement — faster integration of the implant with the bone, lower risk of complications
- Bone augmentation / sinus lift — PRF as a component of biomaterial for GBR (Guided Bone Regeneration)
- Patients with diabetes, smokers, steroid medications — any situations where healing is worse
Apicoectomy — microsurgery endodontics
Apicoectomy is a surgical rescue of a tooth after unsuccessful root canal treatment. Standard sequence: when conventional Re-Endo (retreatment under a microscope) is not enough — we perform surgical removal of the root apex + retrograde fill (closure of the root from the surgical side).
We perform it with an Er:YAG laser (with procedural documentation from my master's thesis at RWTH Aachen). The laser allows for precise incision of the bone + minimal invasion of the periapical tissue. Minimal bleeding. Resorbable stitches. We observe healing radiologically for 6-12 months.
Effectiveness: 70-85% with appropriate qualification (quote from literature, consistent with our results). This is a discipline that Marcin has been working in for years — we combine it with our core microscopic profile (crown imaging, searching for missed canals, laser disinfection).
Supporting technologies
In addition to traditional surgery, we use 3 supporting technologies that accelerate healing and reduce discomfort:
Ozone therapy
Ozone (O3) has strong disinfecting properties — eliminates bacteria, viruses, and fungi without using antibiotics. We apply it to the socket after extraction and around the surgical site. It reduces the risk of infection and dry socket.
Er:YAG laser for incisions
Er:YAG laser (2940 nm) replaces the scalpel for gingival incisions. Advantages: no bleeding (the laser cuts and coagulates simultaneously), no stitches in many cases (the wound closes spontaneously), 2× faster healing of soft tissues.
LLLT — biostimulation
Low Level Light Therapy — low power Nd:YAG laser (0.5W) applied immediately after the procedure. Induces regenerative processes at the cellular level. Patients report significantly less postoperative pain and lower medication consumption.
Postoperative care
Standard recommendations after a surgical procedure. Each Patient receives a written copy + our phone number 24/7 in case of questions. Most recoveries are trouble-free.
- 2 hours without eating/drinking — allow the clot to stabilize. No hot drinks for 24 hours.
- Cold compresses for 15 minutes every 1-2 hours for the first 24 hours — drastically reduce swelling. After 24 hours, switch to warm (accelerate circulation).
- No vigorous rinsing for 48 hours — the clot may detach → risk of dry socket. After 24 hours, gently rinse with saline.
- Medications: ibuprofen 400 mg every 6 hours for the first 48 hours prophylactically. Most Patients after PRF + LLLT do not need stronger medications.
- No smoking for 72 hours — nicotine increases the risk of complications by 3-4×. No alcohol for 48 hours (interaction with medications).
What Marcin brings to surgery
Title: Oral Surgery Academy Graduate (specialized dental surgery program). Plus M.Sc. RWTH Aachen in laser dentistry. These two elements combine into protocols that we use: Er:YAG laser for incisions (minimal invasion), Nd:YAG for hemostasis and LLLT, PRF for every major extraction.
In practice: extraction of a wisdom tooth typically requires a scalpel, forceps, chisel, stitches, 2 weeks of healing with pain. With us: Er:YAG laser instead of scalpel, PRF instead of an empty socket, LLLT after the procedure. The Patient returns to work the next day without pain medications. This is not advertising — it is the standard of our office.
Lectures at the LA&HA Symposium (Slovenia 2019, 2023 + Poland 2022 keynote 'From Root to Crown — Comprehensive Laser Treatment of Teeth'). I teach this protocol to other dentists at international conferences. Every Patient benefits from this knowledge.
Surgery often leads to the next stage
After extraction, it is worth considering an implant instead of waiting for the bone to resorb (2-3 mm per year). Apicoectomy is a surgical rescue after unsuccessful root canal treatment — first check if Re-Endo under a microscope is sufficient.
Frequently Asked Questions (FAQ)
When should wisdom teeth be removed?
When there is not enough space for them, they cause inflammation, cavities in neighboring teeth, or cysts. We assess this through a panoramic X-ray.
What is PRF?
Platelet-rich fibrin from your blood. It acts like a natural bandage, accelerating wound healing by several times.
What are the recommendations after tooth extraction?
2 hours without eating. Avoid hot meals and strenuous activity. Do not rinse your mouth vigorously. Use cold compresses.
Is dry socket a common complication?
It occurs rarely (about 2-5%). By using PRF and ozone therapy, we drastically reduce the risk of complications.
Is wisdom tooth extraction a complicated procedure?
It depends on the position of the wisdom tooth. If erupted — it is a simple extraction, 20-30 minutes. If impacted (horizontal, mesial, deeply embedded in the bone) — 45-90 minutes with root sectioning. Decision after CBCT. Safety is always the most important: when the risk of paresthesia is high, we refer to maxillofacial surgery.
How much pain is there after the procedure?
Most Patients report moderate discomfort for the first 24-48 hours, controlled with ibuprofen 400 mg. Thanks to PRF and LLLT, postoperative pain is significantly reduced compared to traditional surgery. After 3 days, discomfort is typically minimal.
Can I drive after the procedure?
After a standard extraction with local anesthesia — yes, right after the procedure. After inhalation sedation (nitrous oxide) — no, please come with an accompanying person. After more extensive procedures (apicoectomy, implant + augmentation) — we recommend resting that day.
How long does complete healing take?
After a simple extraction: 7-10 days for the gum to close, 6-8 weeks for full bone regeneration. After PRF, soft tissue healing is reduced to 5-7 days. After implantation: 3-4 months of osseointegration before loading with a crown.
Can I avoid wisdom tooth extraction?
Yes, if the wisdom tooth is properly erupted, has contact with the opposing tooth (functions in occlusion), does not cause crowding, and is maintained hygienically. Prophylactic extraction 'because everyone removes them' is an outdated philosophy. Decision after individual assessment + CBCT.
