When to choose an implant?

How the procedure looks step by step

Implantation at Mikrostomart is a process, not just a visit. It consists of several stages, most of which the patient undergoes once, and their effects last for many years.

    Consultation and CBCT

    Medical history, assessment of occlusal conditions, clinical analysis of the gums and bone. We perform cone beam computed tomography (CBCT) — it's like a 3D X-ray that shows the exact bone structure in three dimensions and the precise location of nerves and maxillary sinuses.

    Digital planning

    Based on the CBCT and intraoral scan, we plan in software (3Shape Implant Studio, coDiagnostiX) the exact position of each implant — depth, angle of inclination, distance from nerves and sinuses. The patient receives a visualization and knows exactly what to expect.

    Surgical template

    We print a personalized implant template that fits precisely on the patient's teeth and guides the drill according to the planned trajectory. It's a 'dental GPS' — the implant goes exactly where it was digitally designed.

    Implantation procedure

    The procedure itself, guided by the template, usually lasts 30-45 minutes per implant. In most cases, without cutting the gum and without stitches (flapless). The patient returns home on their own, usually with a mild painkiller.

    Healing and osseointegration

    Soft tissues heal in 2-7 days. The osseointegration itself (the process in which bone fuses with the titanium surface of the implant) takes 2-4 months in the mandible, 3-6 months in the maxilla. The patient has no special restrictions during this time.

    Final crown

    After osseointegration, we perform a digital scan, based on which the lab designs and mills the full ceramic crown. Once screwed or cemented — the implant with the crown functions like a natural tooth.

Three levels of complexity

Implantation does not always mean the same thing. We distinguish three levels of complexity that require different plans and treatment times.

Level 1

Single tooth loss

The most common case. 1 implant, 1 crown. Classic treatment time is 3-4 months. Temporary restoration may be possible immediately in cases with good initial stabilization.

Level 2

Multiple tooth loss — bridge on implants

Several missing teeth in one segment. 2-3 implants supporting a prosthetic bridge. This avoids grinding healthy adjacent teeth. Treatment time is similar to that of a single loss.

Level 3

Full arch — All-on-4 / All-on-6

Patient without teeth or with teeth that cannot be saved. 4-6 implants per arch with immediate loading of a temporary bridge (usually within 24 hours). Possibility to leave the office 'with new teeth' the same day. This is a procedure we have trained intensively — it requires experience, but the result changes the patient's life.

Dental implant structure

A modern implant consists of a fixture (titanium screw in bone), an abutment, and a crown (the visible part). Each component is individually selected to ensure durability and perfect aesthetics indistinguishable from natural teeth.

What happens after the procedure — schedule

Most patients are mainly concerned about what is not visible. Below is a realistic schedule of what happens after the implant is placed.

Day of the procedure

Mild swelling and tenderness in the area of the procedure. Usually, 1-2 painkillers are sufficient. Antibiotic prophylactically for 5-7 days. You can eat after 2 hours (on the non-operated side).

Days 1-7

Soft tissues heal — swelling and tenderness subside on days 3-5. Follow-up visit after 7-10 days (if there were stitches — they will be removed).

Weeks 2-12

Osseointegration — the bone fuses with the titanium surface. The patient does not feel this process, but it is the most critical stage. During this time, no loads on the implant (unless a temporary restoration was planned).

Months 3-4

After confirmed osseointegration — digital scan and crown design. The lab takes 2-3 weeks to complete.

Month 6+

Implant with crown ready. Check-up every 6 months along with hygiene — the implant requires special peri-implant hygiene, which we demonstrate to the patient.

Implantology is a discipline where a mistake costs the patient a lot — sometimes for years. Therefore, we do not shorten any stage. Marcin Nowosielski is a graduate of the Oral Surgery Academy (surgery and implantology), a member of PTSL, and holds a Master of Science in Lasers in Dentistry from RWTH Aachen — this is significant because in selected cases (bone splitting, regeneration of peri-implant gum) we use laser support, which accelerates healing and reduces complications. We work with implant systems that have a solid position in the market (>20 years of clinical documentation) — because an implant is a long-term investment, and there is no room for experiments with unknown brands.

Is the procedure safe?

Yes, we precede it with a detailed interview and CT scans.

Is the procedure painful?

No, it is performed under effective local anaesthesia. Post-procedure discomfort is comparable to a tooth extraction.

How long do implants last?

Properly placed implants with good hygiene can last a lifetime.

Contraindications

Uncontrolled diabetes, pregnancy, severe systemic diseases, age under 16.

Implant or bridge on adjacent teeth — which is better?

In most cases, an implant. A bridge requires grinding down two healthy adjacent teeth (loss of healthy tissue) and does not prevent bone loss in the place of the missing tooth. An implant preserves the bone and does not burden the neighbors. A bridge remains a reasonable option where adjacent teeth require crowns anyway, or when implantation is impossible.

How long does full treatment take — from the visit to the finished tooth?

Standard schedule: 1-2 weeks for diagnostics, procedure, 3-4 months for osseointegration (mandible) / 4-6 months (maxilla), 2-3 weeks for crown fabrication. In total, 4-7 months from the first visit to the finished, full ceramic crown on the implant. In special cases (immediate loading technique), this time can be shortened to a few days.

What if I lack bone under the implant?

There are several options. Minor bone loss — we supplement it during the same procedure with bone substitute material (human, bovine bone, or synthetic hydroxyapatite). Greater loss in the lateral segment of the upper jaw — we perform a sinus lift (raising the floor of the maxillary sinus) as a separate procedure 4-6 months before implantation. In extreme cases — bone autograft from another area or zygomatic implants. CBCT shows which path to take before we make a decision.

Does the implant require special hygiene?

Yes. An implant does not require anything special, but it requires consistency. Daily brushing twice a day, flossing or interdental brushes, regular hygiene in the office every 6 months — this is the standard that a patient with an implant cannot neglect. The most common cause of implant loss after years is peri-implantitis (inflammation of the peri-implant tissues), which develops only in patients with neglected hygiene. At the first follow-up visit, we show exactly how to clean.

Can I smoke after implantation?

Better not. Smoking narrows blood vessels and disrupts healing — in smokers, the risk of implant failure is 2-3 times higher. We recommend at least 2 weeks without a cigarette before and after the procedure. If quitting is not possible — at least significantly reduce during the osseointegration period. This is one of the few things that the patient has a real impact on during treatment.
