Root canal treatment procedure step by step

Complete endodontic treatment protocol under a microscope — from diagnosis to crown placement.

  1. Diagnostics + CBCT (250 PLN + 400 PLN). Interview, pulp vitality test, periapical X-ray, sometimes CBCT for multi-canal teeth. Treatment plan with price ranges and prognosis.

  2. Anesthesia The Wand + rubber dam (15 minutes). Computer-controlled local anesthesia (almost imperceptible), rubber dam isolates the tooth from saliva — sterile working field. Patient breathes freely through the nose.

  3. Opening + locating canals under the microscope (30-60 minutes). Access drilling, locating each canal with ZEISS Extaro 25× zoom. For upper molars, we look for MB2 (the second mesial-buccal canal). Measuring working length with apex locator.

  4. Preparation + irrigation + SWEEPS (45-90 minutes). Reciproc R25/R40/R50 (rotating nickel-titanium files) prepare the canal. Irrigation with 5.25% NaOCl + 17% EDTA. SWEEPS Er:YAG laser (20mJ, 15Hz, 25µs) — disinfection of lateral canals and dentinal tubules. Drying with sterile paper points.

  5. Filling + restoration (30 minutes). Warm gutta-percha + AH+ sealer (or MTA for open apices) fill the entire 3D canal system. Composite restoration with SonicFill. Control X-ray. Recommendation for final crown in 2-4 weeks (protects against fracture).

The procedure is painless — we perform it under local anesthesia using The Wand system (computer-controlled, almost imperceptible). Most patients discontinue pain medication after 24 hours (SWEEPS laser + tight filling reduce inflammatory response). Exception: acute inflammatory conditions (abscess, irreversible pulpitis) — the first 2-3 days may have sensitivity to biting, subsiding after starting antibiotics.

How many visits will full root canal treatment take?

Front teeth and premolars with straight canals: usually 1 visit (90 min). Molars with 3-4 canals: 1-2 visits (each 60-90 min). Re-endodontics: 2 visits with a 2-week break for calcium disinfection. After endodontics, we recommend a final crown within 2-4 weeks (protects against fracture) — this is an additional 1-2 visits.

Why is treatment under a microscope more effective than without a microscope?

Three reasons: (1) Locating MB2 — the second mesial-buccal canal in upper molars occurs in 60-80% of cases, but without a microscope, it is missed in 60% of treatments. Missed canal = chronic inflammation = extraction in 5-10 years. (2) Identifying cracks — microcracks in enamel/dentin visible only in 25× zoom determine whether a tooth can be saved or not. (3) Precision of preparation — preparing to working length ±0.5 mm instead of 'by feel' = better filling = fewer long-term complications.

Does the National Health Fund reimburse root canal treatment under a microscope?

The National Health Fund reimburses ONLY root canal treatment for front teeth (1-3) to a limited extent — without a microscope, without modern disinfection protocols. Endodontics for lateral teeth (4-8) and any endodontics under a microscope are commercial services. This applies to every clinic in Poland. German health insurance may cover part of the treatment costs abroad based on EU Directive 2011/24 — we issue invoices in German.

What guarantee do you provide for root canal treatment?

Endodontics in our clinic has control X-rays after 6 and 12 months (included in the price). If a problem requiring re-endodontics of THE SAME tooth arises within 12 months — we do it for free (with the final crown preserved and regular hygiene visits).
